Appointments and Referrals

Faculty clinical pharmacists in the UCSF Medication Management Service-Fresno meet personally with patients at the Service office by appointment only.

To use our services:
  • Providers complete a referral form for their patients,
  • Patients can receive a referral from they health care provider but can also self-refer. In either case, a completed referral form is required before an appointment can be made.
  • Provider staff or patients can schedule appointments through the internet (see weblink), by telephoning 559-499-6511, or by emailing mms@fresno.ucsf.edu

Note to referring physicians and other referring providers:
Contact the UCSF Medication Management Service-Fresno directly to consult with a faculty clinical pharmacist regarding medication therapy management issues for those patients referred to the Service. There is no charge for these health provider team consultations.

Fees
Fees charged by the Service to patients average from $10 to $60 per visit, depending on the type of service and insurance held by the patient. These fees are discounted by 67% due to support from the UCSF School of Pharmacy.
